Nottingham Inquiry questionnaire

The Nottingham Inquiry is concerned with the events of 13 June 2023 when Valdo Colocane, who had a history of contact with mental health services, brutally attacked and killed Barney Webber, Grace O’Malley-Kumar and Ian Coates in Nottingham city centre, before attacking and driving into three pedestrians, Wayne Birkett, Sharon Miller and Marcin Gawronski, seriously injuring them.  

The Inquiry aims to learn lessons from these tragic events and their surrounding circumstances in order to make recommendations to try to prevent similar outcomes in the future.  

The Nottingham Inquiry invites you to complete this questionnaire if: 

  1. your family member or friend has been killed by a person [“the perpetrator”] who was under, or previously under, the care of mental health services and the perpetrator was criminally convicted for the killing (or was found unfit to plead but nonetheless found to have done the act in question); 
  1. your family member or friend was the perpetrator and was under, or previously under, the care of mental health services and the perpetrator was criminally convicted for the killing (or was found unfit to plead but nonetheless found to have done the act in question). 

The information received in the questionnaires will be summarised by the Inquiry for publication, and will be considered by the Chair when making recommendations. 

Personal data and names set out in the questionnaires will not be published without prior notice.

Scope 

The Inquiry is seeking information about cases where criminal convictions (or criminal findings) have been obtained. The Inquiry cannot investigate any incidents, risks or events that you disclose. If you consider there are safeguarding or criminal matters that require investigation, or you consider that there is a real and ongoing risk of harm to yourself or others, then please contact the police. 

The questionnaire asks for information about:  

  1. the person who was killed (referred to as “the victim”); 
  2. the attack (“the fatal attack”); 
  3. the person who undertook the attack (referred to as “the perpetrator”) and their mental health care; and  
  4. any recommendations you think this Inquiry should make.
